Why Are Prices Increasing?

Like many businesses, we’ve faced steadily rising operational costs over the past year. These include:

  • Higher expenses for content development and expert review
  • Increased server and technology infrastructure costs to maintain performance and reliability
  • Ongoing investments in platform improvements and security


These factors have made it necessary to adjust our prices to ensure we can continue delivering the high-quality resources you rely on for your exam preparation.

COMLEX CDM


COMLEX CDM is a relatively new part of the COMLEX Level 3 exam. CDM is short for "Clinical Decision Making." Essentially, you will be presented with a clinical case and you will need to make decisions about performing a physical exam, ordering labs, interpreting labs, ordering imaging, interpreting imaging, and providing treatment. Treatment could include things like medications, antibiotics, surgery, consultations, fluids, to name a few. 


There are two different types of answer types when answering questions: There is the multiple choice answer where you have a number of choices and you can order a specific number of answers (i.e. like ordering 3 of 15 answers). The second type of answer is the short answer or fill-in-the-blank answer. This is where you can type anything in the answer. COMPLEX CDM is very specific on how they wish for the fill in the blank to be formatted. Check out our post on how to correctly fill out the fill-in-the-blank answers. There are quite a few rules that the COMLEX has posted. 


Each COMLEX CDM case has 2-4 questions on each case and can ask anything that has been listed above. We go over all aspects of a case so that you can answer any of the questions that can be asked for a specific case, so our cases have more questions that you will typically see on the COMLEX. Our cases on cdmcases.com will have between 4-8 questions to cover all aspects of the case which include the appropriate part of the physical exam that should be ordered, the appropriate lab work and imaging, appropriate treatment, and appropriate follow-up and required consultations.

This is a significant portion of your grade that you should not neglect. This can be the difference between passing and failing. The software takes some getting used to, so you should be doing many many practice cases. No one offers as many cases to practice for the CDM portion of the exam.

We have 100+ COMLEX Level 3 CDM cases that will go through the most commonly seen cases on the COMLEX. It will go through physical exam findings, ordering tests, data interpretation, generating a diagnosis, and treatment. 


Treatment includes prescribing medication, planning follow-up, and counseling.


Each cases will have 2-4 questions. We have more questions in our cases because a clinical case may focus on a particular part of the case and not others. We include all parts so that you will be prepared for any COMLEX Level 3 CDM cases question in regards to a specific case.  


There will be 26 cases on the morning of the second day of the COMLEX test. There will be 13 cases followed by a 10 minutes break and then another 13 cases for a total of 26 cases. Each case will have 2-4 questions for a total of approximately 70-80 questions total. The multiple choice questions for the rest of the test will be 420 questions (6 sections of 70 multiple choice questions = 420 questions), so with the CDM cases there will be a total of 490-500 questions. 


As this is 2 sections out of 8, this will be approximately 25 percent of your grade, so it is important to study hard for the CDM portion of the exam. What are CDM Cases?


CDM Cases are short for Clinical decision making cases. This is a relatively new part of the COMLEX Level 3. Essentially, there will be a case introduction with a clinical scenario such as a patient presenting with chest pain. Some things that the case can ask you in the CDM Cases is what physical exam to perform, what imaging to get, what medication to prescribe, the disposition of the patient (discharge home or admit to the hospital), and the diagnosis for the case. 


There will be 26 of these cases which will take 3.5 hours on the morning of the second day of the COMLEX Level 3. The best strategy to do well on these cases is to know everything about that particular case so that you will be prepared for anything that the exam will throw at you. Knowing the correct test, imaging, medications, and diagnosis is what will make you do well on a particular case. 


The key to doing well on the CDM Cases portion of the test is to practice, practice, practice. The software can take some getting used to, particularly the fill in the blank (the test calls it short answer responses). This has quite a bit of rules which you will need to be familiar with prior to taking the test.

What are the rules for the short answer questions on the CDM portion?

Rules for short answer (fill in the blank):


#1 - If giving a drug, give drug name, not classes (example: give hydrochlorothiazide rather than "antihypertensive").

#2 - If giving a drug, give the route of the drug : Oral (by mouth), inhalation, sublingual, intravenous, intramuscular, subcutaneous, intradermal, topical) Try not to abbreviate.

#3 - Imaging studies: Must include anatomic site (example: Chest xray instead of xray)

#4 - Avoid or limit use of abbreviations due to lack of standardization. We accept some standard abbreviations, but it would be safer to spell it out and not abbreviate on the test!

#5 - Protocols will not be accepted as correct because they are often not universal.

#6 - Urgency: Include “stat” when a procedure or diagnostic test is needed within 30 minutes, regardless of clinical setting, including the emergency department.

#7 - Referrals and consults: If the correct plan of action requires a specialty referral and/or consultation, in addition to the type of specialist you must specify the appropriate procedure or management step the specialist will perform (for example, “refer to psychiatry for cognitive-behavioral therapy”).


Note that just like in the real test, there might be 2 phrases required in a fill-in-the-blank answer. As with the example above with referral to psychiatry, the first phrase that will be matched is "psychiatry" and the second phrase that will be matched is "cognitive behavioral therapy." In the feedback section, there will be a marker "Additional phrase required" which will show you the second phrase that must be matched.


Another example of 2 phrases that must be matched is with fill in the blank medications which require the route of administration. The first phrase would be the medication such as lisinopril. The second phrase would be the route which would be oral or by mouth or PO (though try to limit abbreviation usage). The correct response would be lisinopril by mouth.

This is a lot more intuitive then the fill-in-blank or short answer type questions. The question will tell you how many options you can select. There is a built in stupid test in this type of question. If you select more options than they tell you can, you will GET A ZERO for the entire question, so DONT DO THIS! For example, if they say you can select up to 5 of the 15 options, and you select 6 options, then no matter what you have selected, you will get a zero for that question. If you did this, then you won the stupid test. This was very stupid thing to do. It's easy not to do this, so just don't do it. You select the BEST answers that would pertain to the question.


That's pretty much it. There isn't much else to it. Select the best responses, make sure that you don't go over the allowed responses, and make sure you read all of the options before you go to the next question.

How is the Clinical Decision Making (CDM) Section Graded?


There are 26 CDM Cases on the COMLEX which means that there will be 70-80 questions total (some tests have more or less questions depending on the draw from the Qbank). Each case has 2-4 questions to answer.


The CDM portion will take 3.5 hours (2 sections). There are a total of 8 sections on the entire COMLEX, so 2/8 = 25% of your grade.


For Extended Multiple Choice, partial credit can be awarded if you answer some but not all of the correct answers (for example, the question has 5 correct choices, and you select 4). You will receive a zero if you select more choices than allowed (for example, you select 7 answer choices when only 6 were allowed). You will receive a zero if you select an answer choice that would harm the patient.


All answers are weighted equally (one question is not worth more than another question). The Short Answer responses will get a point if you enter the correct response. You will get a zero on the short answer response if you put more than 1 response in a single box (for example: IV fluids and administer lisinopril). You will also get a zero for that question if you enter a response that could harm the patient.


This is an important part of Level 3, so this section should be taken seriously as 1/4th of your total grade which can make or break your score. We have released our beta version of CDMCases.com! We have completed 75 cases and are going to be doing the beta version until we reach 100 cases. Our goal is to reach 140 cases by the end of the year. The beta version means that you might encounter some minor issues that still need to be fixed before we release the full version. In compensation for trying our beta version, it will only be $1.


Why charge a $1? This is simple psychology: if you are willing to pay even a token amount, these people tend to be more involved in the process. This makes the process cleaner. We have a limited amount of beta testers, so it is a first come first serve process.
